Trans Modulator or Fluid? Mounts even?

My trans Shifts into gear Really hard and has a delay of 2-3 seconds ONLY when going into gear, such as Drive or Reverse, but not when driving.  The shifts arnt really hard when driving around. when going into gear its so hard that the car will jump if the brakes arnt on or on hard enough. If i set the parking brake, the car takes alot of gas to move. But With it on, and i shift it, the car will jump a foot! thats a little extreme if you ask me! Whats up?

I would start with a new hydraulic motor mount.
