A gm dealership  :D

Seriously, now that i'm a jackass, thats the only place to get one. The are so commonly destoryed by road hazards you probably won't find a good used one, but no one aftermarket makes one.

How much do they cost and do they come painted?

I have had a couple of BIG girlfriends in the car since you last saw it. Of course I have only had them in the car at night when no one could see inside!!!! 8)

Only if your car is black!

They come with a flat black primer over the yellow plastic. Its not too hard to get good results with spray paint from the touch up section of your parts store (thats how I did mine, and it came out pretty good). I've heard prices anywhere from $90 to $180. A decent price is about $125, if you have a hookup you can probably get the $90 price.
